Please make a copy of everything you turn in.

• Part A and B are to be completed at least annually and can be completed by parents.
• Part AB and C needs to be completed by a doctor and is needed before the Scout goes to any event over 72 hours such as Boy Scout Summer camp and redone annually.(includes part A and B)
• BSA HIGH ADVENTURE physical is needed if he goes to any BSA HIGH ADVENTURE BASE. You will be provided this form if you need it.
